Home and Community Care Support Services South East is one of 14 Home and Community Care Support Services organizations in Ontario with a focused mandate to deliver local health care services such as home and community care and long-term care home placement.

These organizations were previously known as Local Health Integration Networks (or "LHINs") at a time when they also led local health care planning and funding, and those functions are now part of Ontario Health.

Home and Community Care Support Services South East is dedicated to ensuring the ongoing delivery of local services while Ontario makes changes to improve the health care system to give patients better-connected care with health care providers working as one coordinated team in Ontario Health Teams.
The south east region extends from Brighton on the west, to Prescott and Cardinal on the east, north to Perth and Smiths Falls, and back to Bancroft. The boundaries are for funding and planning purposes only. Residents of the Home and Community Care Support Services South East can seek health care services wherever they prefer, inside or outside these boundaries.

Position Summary

Are you an experienced registered nurse with a Masters level education in Wound Healing and Tissue Repair seeking a rewarding career that cares for others, in a professional practice that cares for you? You’re looking in the right place.

As an Advanced Practice Nurse – Wound Ostomy and Continence, you will contributed to excellence in the delivery of care to clients requiring support with wound care, ostomy management and continence care within the community serviced by Home and Community Care Support Services South East. You will be collaborating with inter professional care teams across the health care continuum and provide direct care for individuals in collaboration with a care team. You will also provide face to face and virtual visits to patients to collaborate and conduct comprehensive clinical assessments to develop a care plan and set therapeutic service goals.

What will you do?

• Provide face to face and/or virtual visits with patients to collaborate and conduct comprehensive clinical assessments, and consults with patients and other team members as required, to develop a care plan and set therapeutic service goals
• Provide clinical expertise/advice to primary care physicians and community nurses.
• Provide input and evidence informed clinical rationale with respect to reassessment and adjustment to care plans as necessary based on patient progress.
• Collaborates with discharge planning for patients, when therapeutic service goals have been met.
• Teaches and delegates specific aspects of patient care to a person(s) deemed competent and capable to carry them out and accepts responsibility for these delegated tasks according to the standards of the College of Nurses of Ontario and Nurses Specialized in Wound, Ostomy and Continence Canada
• Collaborates with the patient/family and inter-professional team to design goals that best matches the patient’s circumstances.

What must you have?
• Current unrestricted registration with the College of Nurses of Ontario as a Registered Nurse
• Certificate as a Nurse Specialized in Wound Ostomy and Continence Canada Program (Previously known as Enterostomal Therapy Nursing Program).
• Masters level education in Wound Healing and Tissue Repair required.
• Minimum 3 years’ experience in WOC service delivery.
• Hold current CPR-Level C &AED certification.
• Vehicle and a valid Ontario driver’s license is required.
• A thorough understanding and working knowledge of the Canadian Standards of Practice for Nurses Specialized in Wound, Ostomy and Continence.
• Knowledge about leading priority interventions for chronic wound care, and implementing the transferable knowledge to SPOs and patients. e.g., compression therapy, DFU offloading, and pressure redistribution practices.
• Thorough knowledge of wound, ostomy and continence products and indications for use
• Solid knowledge of health care related legislation and practices.
• Knowledge of direct care / case management models used in community health care organizations.
• Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
• A valid driver’s licence and access to a reliable vehicle
• Proficient in a Windows environment
